Abstract

Long-term oral therapy with levodopa is associatedwith the development of motor fluctuations and dyski-nesia in a large percentage of patients with Parkinson’sdisease (PD). Motor complications are associated witha number of non-motor symptoms and have a negativeimpact on disability and quality of life. There are threetherapeutic options available for the management ofpatients at this advanced stage: high frequency deepbrain stimulation, continuous subcutaneous infusionof apomorphine, and continuous intestinal infusion oflevodopa/carbidopa. On the basis of published dataand in consideration of the risk-benefit profile of cur-rent therapeutic strategies, we here propose an algo-rithm to help clinicians select the most suitable treat-ment option for patients with advanced PD.
