Abstract

Objective: During the past decade, we encountered an increasing number of young patients with esophageal and cardia cancer. Surgical resection is the mainstay of the treatment, but it is no clear whether young patients have more aggressive course and worse prognosis. The aim of this retrospective and comparative study is to evaluate the clinicopathologic difference and survival between young and elderly patients. Methods: From 1992 to 2007, totally 894 patients with esophageal and cardia cancer received esophagectomy with reconstruction at our institution. Group I included 109 patients aged 50 or younger and group II included 785 patients aged over 50 years. These two groups were analyzed and compared according to sex ratio, comorbidity, tumor characteristics, treatment, pathologic findings and outcome. Results: Cardio-vascular and respiratory disease and ASA risk resulted more frequent in elderly patients while no difference was found evaluating istological type, grading, tumor site and history of Barrett’s esophagus. Young patiens had worse but not statistically significant stage at diagnosis and in these patients neoadjuvant treatment were more frequently applied. Pathological stage, surgical outcome and survival resulted similar in the two groups of the study. At multivariate analysis of survival age did not show any prediction. Conclusion: Young patients with esophageal and cardia cancer have minor comorbidity respect to elderly patients but similar outcome after esophagectomy.
